Guidelines for Submitting a Case Study

The tabs on the submission form allow you to provide a general summary of the project, explain the planning process you went through, give details of what you did and the outcomes of your project, and finally upload any additional information in the form of reports, articles or links to external websites.
Help is provided throughout the form, with hints and tooltips in each section. If you require further assistance you can also send a query to support@evolvingpractice.org.
Tab 1: About the Project
- Project title
- Project start and end dates
- Local area in which the project took place
- Health or social care setting (such as primary, acute or community care)
- Equality groups that your work targetted
Tab 2: Planning
- Aim and objectives, including what you wanted to achieve and the steps you planned to take. (Character limit: 500)
- Guidelines or reference documentation that you followed (Character limit: 500)
- Capacity building, training or advice that you and/or your staff received (Character limit: 500)
- Approaches you used to involve the public
Tab 3: Outcomes
- Description - the practical steps you took to ensure the project made things better for patients and the public (Character limit: 1000)
- Evaluation - how do you know your project made a difference to patients and the public? (Character limit: 750)
- Future plans - how will you build on this experience? (Character limit: 500)
- Image - you can supply an image of your project (such as a logo) or a photograph. Please ensure you have the consent of everyone who appears in your picture.
Tab 4: More information
- Associated documents, such as a newsletter, report or examples or documents you have produced
- Useful websites which mention your project, or which contain relevant information
- Contact name
- Contact job title
- Contact email
- Enable comments - tick the box if you would like other users to contact you for more information. We will not disclose your email address.
